Can I be turned down for this insurance?
You cannot be turned down for coverage if you are age 18 to 70 (18
through 64 in Minnesota) and aren't already insured under other
Accidental Death Insurance from Mutual of Omaha.

Does this Mutual of Omaha Accidental Death Insurance replace my
current life insurance?
This insurance pays benefits regardless of any other policies you may
have. It makes a good addition to any other kinds of insurance you
have and is a less expensive way to add coverage for accidental
death. As a matter of fact, you can add up to $1,000,000 to the
total you leave your family if you should die in a covered accident
while this coverage is in force.

Will my rate go up over time?
Your rate will increase only if the same change is made for all
insurance of this Form issued to Insureds of the same class in the
same state.

Can I cover my family members also?
Yes. The Family Plan insures you, your spouse*
18 to 70 (18 through 64 in Minnesota), and all unmarried dependent
children under the age of 21 (19 in New Jersey, 22 in North Dakota,
24 in Indiana, 25 in Texas, 26 in Utah), or under age 25 (age 24 in
Lousiana, 26 in North Dakota and Utah, no age limit in Texas) if a
full-time student at an accredited educational institution. Those
insured may have no other accidental insurance with Mutual of Omaha
Insurance Company.

If I have family coverage and both my spouse*
and I are killed in a covered airline accident, what is the maximum
my beneficiary can receive?
The benefit will be paid on you and your spouse*.
For example, if you have a $1 million policy, your beneficiary will
receive $1.5 million.

What if I get this insurance and then decide I don't want it—do I get
my money back?
If you return it to us within the first 60 days of the Effective Date
(30 days in New York), you will receive a complete refund. Your
Effective Date will be shown on the insurance when you receive it
in the mail.
